This project will examine the mechanisms underlying innate resistance to Mycobacterium tuberculosis infection among heavily exposed tuberculosis (TB) contacts in Indonesia. Each year, millions of people worldwide become infected with Mycobacterium tuberculosis. However, some people, even when heavily exposed to an infectious TB patient, do not get infected. They almost certainly inhale M. tuberculosis but contain or clear infection through innate immune mechanisms. We studied this phenomenon among heavily exposed household members of infectious TB patients in Indonesia. BCG vaccination exerted strong protection against M. tuberculosis infection, and those contacts who were protected had altered innate immune cell distributions and cytokine responses. This led to the hypothesis that BCG-induced 'trained innate immunity', mediated by epigenetic and metabolic reprogramming of innate immune cells, contributes to 'early clearance' of M. tuberculosis, and that trained immunity can be harnessed to help protect against M. tuberculosis infection.
